Applicants need to submit the following documents:
- Application Form for Certificate of Residence at Youth Correctional Institution (can be collected at the service application location or downloaded from the Correctional Services Bureau’s website);
- Copy of valid identification document(s);

When copies are submitted for application, originals must be presented for verification.

Applicant can choose to collect the Certificate either at Youth Correctional Institution or Correctional Services Bureau Service and Information Centre when lodging the application.
